Quadratic formula practice

Every question gives a quadratic equation with integer roots. Apply the quadratic formula $x = \dfrac{-b \pm \sqrt{b^2 - 4ac}}{2a}$, then enter both roots — smaller one first. You get full credit only if both roots are correct.
